Motor Accident

• Photographs of vehicle in its damaged state
• Estimate of repairs
• Receipt for towage of service if any
• Comprehensive report of the accident by the driver involved
• Police report (if involving third party bodily injury/death
• Evidence of third party insurance
• Copy of drivers licence

Motor Theft

• Police report (interim & final)
• Original receipt of purchase
• Original insurance certificate
• Vehicle registration documents with licensing authority
• Evidence of anti-theft device installation
• Original vehicle licence
• Original way bill
• Log/hand book
• Both sets ignition, boots & door keys
• Extract of police diary
• Sworn affidavit (in case of loss of document(s)
• An advert in popular news paper (if driven away by driver)

Fire

• Estimate of repairs
• Fire brigade report on the fire
• Invoices and other evidence to support the existence of damaged goods
• Stock records card / stock taken prior to and immediately after the loss
• Eye witness account
• Photographs of the incident  

Burglary

• Police report
• Report of security guard/personnel on duty
• Statement of claim showing breakdown of stolen items/goods
• Invoices/purchase receipt of stolen items
• Stock taken prior to and immediately after the loss 

Workmen Compensation (Injury)

• Medical bill/doctor’s report awarding % of disability
• Evidence of salary/wages for 3 months prior to accident
• Excuse duty/sick leave certificate
• Discharge certificate

Workmen Compensation (death)

• Death/burial certificate
• Coroners inquest/post mortem report
• Evidence of salary/wages for 12 months preceding death
• Application filed to a court of law or letter of administration 

Group Personal Accident (Injury)

• Medical bill/doctors report awarding % disability
• Evidence of salary/wages for 3 months prior to accident
• Excise duty/sick leave certificate
• Discharge certificate

Group Personal Accident (Death)

• Death/burial certificate
• Coroner’s inquest/post mortem report 

Cash in Transit

• Police report
• Security personnel report
• Statement of loss
• Cash book/bank statement
• Estimate of repairs for damage safe (if any) 

Goods in Transit

• Police report
• Suppliers invoice
• Way bill/delivery notes
• Statement details of claim
• Copies of correspondence exchanged between the consignee and the transport – Hired vehicle 

Marine (Cargo)

• Survey report
• Original certificate of insurance
• Original form T.38
• Original bill of lading
• Original supply of invoice
• Original way bill
• Statement of claim
• Short landing/discrepancy certificate
• Original or copy of shipping invoices, specification and or weight notes at final destination
• The vessel’s out-turn report

Marine (Hull)

Engineering
